Description

The Resource Navigator Volunteer assists Hilltop staff members in supporting youth and young adults in crisis intervention and emergency sheltering needs.  Under direction of Hilltop staff, uses a variety of evidence-based practices to help youth and young adults navigate the resources to become healthy, stable, safe, connected and housed

Basic Requirements:

·       Completion of volunteer application and interview.

·       Successful completion of appropriate screenings (Hilltop Non-Residential Criminal Background check without CAPS or MVR, TRAILS Child Abuse Registry screening, and finger printing)

·       Completes volunteer and program orientation and position specific training.

·       Ability to work with diverse populations

·       Dependable, responsible, and consistent.

·       Excellent verbal and written skills.

·       Excellent interpersonal and customer service skills.

·       Strong analytical and problem-solving skills.

·       Maintain client and family confidentiality.

·       Minimum age requirement: 18 or 14 with adult supervision

·       Completes and submits volunteer hours in Get Connected by the 5th of each month.

·       Abide by all Hilltop policies/procedures and Code of Conduct.

 

Key Responsibilities:

Under the guidance and supervision of Hilltop Connections staff:

·       Become trained and/or certified in assigned Hilltop Program, curriculum or Practice, and conduct duties in accordance to fidelity standards, grant requirements and program guidelines.

·       Complete certification(s) required for position, which may include, but not be limited to training for peer support, and therapeutic case management. Credentials of the Family Resource Center Association (FRCA), housing system /programming models, or other required training as applicable to assigned role.

·       Make initial contact via walk-ins, internal or external referral sources and /or contact through street outreach, with participant to present available services and provide referrals and/or concrete support through one-on-one meetings and/or phone contact.

·       Upon enrollment, create a working file and/or complete data entry with all necessary documentation: assist participant in developing their individualized goal/plan for safety and housing.

·       Administer all required assessments and screenings with participants.

·       Refer participants to external services and resources when the needs are outside the scope of Navigator’s professional training.

·       Create and maintain accurate and timely contact notes and enter timely data in appropriate program databases per program regulations.

·       Collaborate with relevant agencies to assist participants with meeting goals. Contact parents and/or DHS and have appropriate permissions to work with unaccompanied minors.

·       Plan, advertise and implement lessons, activities and classes based on specific program curriculum requirements. Provide mentorship.

·       Accompany, facilitate and/or attend meetings/staffing with court, school, medical and/or legal related agencies.  Provide or brainstorm ideas regarding transportation, as needed.

·       Facilitate screenings for behavioral health needs, health needs, resource needs and housing to complete appropriate documentation and processes as needed or if required by program.

·       May share “lived” experience with participants by modeling effective personal interactions and behaviors, advocating for and supporting individuals, sharing personal experiences, mentoring individuals, and helping to build natural supports, if available.

·       Facilitate a POD.

·       Assist with monitoring/ordering facility supplies.
