Hilltop Residential Programs
Life Adjustment Program
- Hilltop Brain Injury Services provides assisted living support for adults with brain injuries through the Life Adjustment Program (LAP). This long-term, supported housing community is nestled in the heart of Grand Junction, Colorado. Bacon Community Campus sits on eight acres of green, park-like grounds. LAP provides a vibrant and diverse option of activities that connect residents to the greater community. Our kind staff promotes choice, dignity, and encourages independence through individualized care-plans for all residents.
The Commons and The Fountains Assisted Living Communities
- The Commons and the Fountains offer gracious living, personalized care, and unsurpassed amenities in a tranquil neighborhood. They provide comprehensive and compassionate services including independent and assisted living options, memory care, wellness programs, and short-term stays.
The Cottages Independent Living Community
- The Cottages offer independent living for older adults while giving them the freedom to make the most of their retirement.
Senior Daybreak
- Senior Daybreak is a unique day program that keeps seniors with memory loss safe and secure while providing their caregivers the flexibility and support they need to care for their loved ones at home. They also offer valuable information and support groups specifically designed for those caring for someone affected by dementia and Alzheimer’s.
